Statewide Tests: Annual Notice and 30-Day Notice with Opt-out Form
In Oregon, students in grades 3-8 and 11 are expected to participate with state testing each spring. The Oregon Student Assessment Bill of Rights allows parents and adult students to opt out of state assessments. This page provides information and opt out/exemption forms.

Annual Notice for Statewide Tests
The Annual Notice for Statewide Tests is published by ODE at the beginning of each school year. This notice informs parents of state testing requirements, why participation matters and parent and adult student rights regarding state testing. These copies are provided for reference:
If a form is submitted after a student has begun testing, the student will stop testing and the District Test Coordinator will work with ODE assessment staff to invalidate the test.
