Review the Leave Types page to figure out which leave applies to you and what vendor to contact to request your leave. Please note that if a leave is eligible for both Paid Leave Oregon and FMLA you may either apply for Paid Leave Oregon (FMLA runs concurrently when taking PLO) or FMLA only.

• Reason for leave and type of leave.
  • Reasons: medical, family medical, pregnancy disability, parental bonding, safe leave. 
  • Types: intermittent, reduced work schedule, or continuous leave.
     
• Your provider’s name, name of the facility they work at, their phone number, and their fax number.
  If AbsenceResources has permission from you to do so, they can outreach to your provider on your behalf.
   
• The Group Policy Number: 762236

Staff may choose between using Zero, Half, or Full Leave Balances. Please note that responses received on or after the 10th of the month may not be processed until the following payroll period. Complete and provide necessary documentation for your leave to AbsenceResources and/or The Standard. If you requested leave for your own or a family member's medical condition, work with the provider to complete the required medical certification. 

• If you are insured by Kaiser, work with the Release of Information (ROI) department to complete your medical  certification. More information is available here.

AbsenceResources and Standard will send communications via email or U.S. mail to update you on the status of your request. To ensure that there are no delays reviewing your leave request, please review any communication you receive from either vendor.

If your leave has already started be sure to report absences to AbsenceResources and The Standard. For The Standard, you must report your absences for all leave frequencies (continuous and intermittent both) in order to be paid. Absences may be reported in advance. If you are missing time intermittently, you are required to follow call out procedures and notify your site when you utilize your intermittent leave.

At least two weeks before your planned return to work, confirm your return to work date with: PPS Leaves Team (leave@pps.net), Your Supervisor, The Standard, and AbsenceResources.
