Community Transition Program

  • Field of snow Inclement Weather

    • Watch the news in the morning, or check the district website for school closure information. If Portland Public Schools is closed the worksites are closed (AM and PM).
    • If  Portland Public schools is opeing two hours late, worksites will be closed and students will be transported to their classroom instead of worksites. At the end of the day, the students will be transported from the classroom to home. (PM worksites will also be closed if school is on a delayed start in the morning.)
    • If the student is unsure about where to report due to school closures, they can call the worksite leader, or SE main office at 503-916-5817

  • We serve young adults, ages 18-21 who have an Individualized Education Plan, and graduated from high school with a: modified diploma, extended diploma, certificate of participation, or a certificate of completion. 


    Our mission is to support young adults in their transition to life after high school. Our key focus is to help students achieve the greatest degree of independence and quality of life with an emphasis on connecting students to appropriate community and adult services. Our students learn valuable self advocacy skills and form meaningful relationships.
