2018-19 30-Day Notice for Statewide Tests and Opt-out FormParent Request for Opting Out or Exempting Students from Oregon Summative Assessments
- Parents must return completed forms to the school office.
- Incomplete or modified forms will not be accepted.
- Parents may not write in additional assessments to the state opt-out form.
- Forms will be accepted at anytime. Parents are encouraged to submit forms by February 1.

Oregon Science and ELPA Summative Exemption Request FormIf parents change their minds after opting students out of testing and decide to have students test, parents should complete an opt-back-in form. Testing Help will unblock students once a signed form is received.
