• The Oregon Department of Education has submitted a summative test waiver request to the US Department of Education. If that waiver request is approved, ELA, Math and Science testing will not happen this year and opt-out and exemption forms would not be required. If the waiver is not approved, then summative tests may be expected if PPS feels it is safe and equitable to provide testing opportunities.
     
    Following are the state-required 30-Day Notice for Statewide Tests and opt-out forms. If a parent wishes to submit an opt-out form now, rather than wait until waiver approval, we recommend they complete an electronic opt-out at this link rather than the traditional PDF form.

    Parent Request for Opting Out of Oregon Summative Assessments (ELA and Math)
    • We recommend using this link to submit a digital opt-out form in spring 2021. The digital form is available in multiple languages (select the desired language from the dropdown list in the upper right hand corner).
    • If you wish to use the PDF opt-out form:
      • Email completed forms to your teacher
      • Incomplete or modified forms will not be accepted.
      • Parents may not write in additional assessments to the state opt-out form.
      • Forms will be accepted at any time.

    If a form is submitted after a student has begun testing, the student will stop testing, but items answered up to that point will be scored and the student will receive a performance level and/or overall score.
