    Initial Claim Form Process & Instructions

    PLEASE NOTE: AFTER COMPLETING THIS PROCESS YOU MAY RECEIVE AN EMAIL OR PAPER MAIL INDICATING APPROVAL OR DENIAL. YOU DO NOT NEED TO DO ANYTHING WITH THIS INFORMATION. THE LETTER IS AUTOMATICALLY GENERATED. THE OREGON EMPLOYMENT DEPARTMENT WILL STILL PROCESS YOUR CLAIM VIA THE WORK SHARE PROGRAM AS SUBMITTED BY PPS.

    Employees who did not submit their Initial Claim Form online prior to Thursday, May 21 deadline must now complete the paper form. The online option is no longer available. 

    Review the information below before beginning the claim form.  You will only need to complete this process one time. Some eligible employees will be required to submit information about additional income, retirement, or leaves of absences - visit this website for more information. 


    To avoid additional delays, the Initial Claim Form should be completed as soon as possible. 


    Note: If you submitted an Unemployment claim in the past two months, regardless if it was related to PPS or another employer, you do not need to complete this process.

    Tips prior to starting your application:

    • Be prepared with the following information:
      • Social Security Number.
      • Start date with PPS.  If your employment with PPS started before November 1, 2018, an estimated date will be adequate. 
      • Work history for the last 24 months, including dates of employment, your employers' business names, addresses and phone numbers. 
      • Estimated salary and total income from each employer.
    • You can find your hire date in PeopleSoft Employee Self Service (ESS). 
      • Remember, you must be on the PPS network, connected to VPN, or have set up two-factor authentication in order to access Employee Self Service.
      • If your employment with PPS started before November 1, 2018, an estimated date will be adequate. To look up your start date: Personal Details > Additional Information, use Last Start Date if you have one, otherwise use Original Start Date.

    • You will be asked about other employment. You must answer this honestly - even if your work with the other employer has not changed. 

     

    Completing the Initial Claim Form - THE ONLY claim you will file for the PPS Work Share Program

    • Review these instructions on completing the form.

    • After you complete the form email it to hronline@pps.net with the subject line: “Completed Work Share Claim Form”.

    • Tax Withholdings: If you would like to have taxes withheld from your payments, complete the Tax Withholding Form available here: English | Spanish

    • Payments: The first payment will be a check from the Oregon Employment Department, the following payments will be via a Visa ReliaCard. 
      • If you would like to set up Direct Deposit, complete this authorization form (English | Spanish) and submit directly to the Oregon Employment Department. You can view additional information on Direct Deposit and ReliaCard on the OED website here.
        • The Direct Deposit form has a field called, "BYE". This stands for Benefit Year End. You can enter 5/3/2020.
